💉 OTC Medications

Acetaminophen is the go-to pain reliever during breastfeeding. It's one of the most studied and consistently recommended OTC medications for nursing moms.

Afrin nasal spray is safe during breastfeeding because it works locally with very low systemic absorption. Limit use to 3 days to avoid rebound congestion.

Magnesium hydroxide antacids (Maalox, Milk of Magnesia) are safe during breastfeeding. They work locally in the GI tract with minimal systemic absorption.

Non-bismuth anti-diarrheal products are safe during breastfeeding. Kaolin-pectin preparations work locally and have no meaningful systemic absorption.

OTC antifungal creams are safe during breastfeeding. They're minimally absorbed topically and are actually used to treat nipple thrush in breastfeeding moms.

Aspirin at full doses is not recommended for pain relief during breastfeeding. Low-dose aspirin (81mg) as prescribed for cardiovascular or clotting conditions is acceptable.

Topical Benadryl cream (diphenhydramine) applied to small areas is safe while breastfeeding. Very little absorbs into the bloodstream from skin application. Avoid applying it on or near the breast area where your baby might ingest it orally. For large areas of itching, oral antihistamines like cetirizine or loratadine are preferred.

Calamine lotion is safe during breastfeeding. It's a topical product with negligible absorption used for itch relief from rashes and insect bites.

Cetirizine (Zyrtec) is generally preferred over older antihistamines like Benadryl during breastfeeding, but there's a small risk of infant drowsiness and it may reduce milk supply in some moms.

Cough drops are safe during breastfeeding. They work locally in the throat and systemic absorption is negligible.

DayQuil contains acetaminophen (safe) and dextromethorphan (safe), but also pseudoephedrine or phenylephrine as a decongestant, which raises supply concerns.

Dextromethorphan, the cough suppressant in Robitussin and many cold medicines, is considered safe during breastfeeding at recommended doses.

Use Benadryl with caution — it can cause sedation in your baby and may reduce your milk supply. For allergies, loratadine or fexofenadine are better choices.

Dramamine (dimenhydrinate) should be used with caution while breastfeeding. It's an antihistamine that may cause drowsiness in your baby and could potentially reduce milk supply. For occasional motion sickness, a single dose is likely fine. For frequent use, meclizine (less sedating) may be a better option. Avoid long-acting formulations.

Excedrin contains aspirin plus caffeine plus acetaminophen — the aspirin component makes it a caution during breastfeeding. Occasional single doses are low risk, but it's not the first choice.

Pepcid (famotidine) is safe during breastfeeding and is the preferred H2 blocker for nursing moms. It transfers minimally into breast milk.

Fexofenadine (Allegra) is considered safe during breastfeeding. It's non-sedating and transfers into breast milk in very small amounts.

Flonase is safe and actually the preferred treatment for nasal allergies during breastfeeding. It acts locally in the nasal passages with minimal systemic absorption.

Gas-X (simethicone) is completely safe while breastfeeding. Simethicone is not absorbed into the bloodstream at all — it works entirely within the GI tract by breaking up gas bubbles. Since it never enters your blood, it cannot enter your breast milk. It's so safe that it's even given directly to infants as Mylicon drops.

Guaifenesin (Mucinex) is considered safe during breastfeeding. It's an expectorant that loosens mucus and has minimal transfer into breast milk.

Hemorrhoid creams are safe during breastfeeding. Postpartum hemorrhoids are extremely common, and topical treatment with Preparation H or similar products has negligible systemic absorption.

Hydrocortisone cream is safe during breastfeeding. Topical application results in minimal systemic absorption, so little to none reaches breast milk.

Ibuprofen is one of the preferred pain relievers for breastfeeding moms. It's highly protein-bound so very little reaches your milk, making it exceptionally safe.

Imodium (loperamide) is safe during breastfeeding. It's poorly absorbed and very little reaches breast milk, making it a good option for diarrhea relief.

Loratadine (Claritin) is the preferred antihistamine during breastfeeding. It's non-sedating, transfers minimally into breast milk, and has a good safety track record.

Meclizine (Bonine, Antivert) is considered compatible with breastfeeding and is preferred over Dramamine for motion sickness while nursing. It's a less-sedating antihistamine that causes less drowsiness in both you and your baby. Limited drug transfer into breast milk has been reported.

Melatonin supplements while breastfeeding are a gray area. Your body naturally produces melatonin, and breast milk contains melatonin that varies with your circadian rhythm (higher at night). However, supplemental melatonin doses (1–10mg) are far above physiological levels, and the effect on breast milk melatonin content and infant sleep regulation is not well studied. Occasional low-dose use (0.5–1mg) is probably fine.

MiraLAX is safe during breastfeeding. It's an osmotic laxative that draws water into the colon and is not systemically absorbed.

Naproxen is generally considered safe but is a second-choice NSAID compared to ibuprofen during breastfeeding, because it has a longer half-life and slightly more exposure through milk.

Neosporin is safe during breastfeeding. It's a topical antibiotic ointment with negligible systemic absorption.

NyQuil contains multiple active ingredients including alcohol and often antihistamines — not ideal for breastfeeding. It's better to treat each symptom separately with single-ingredient products.

Omeprazole (Prilosec) is safe during breastfeeding. It's a proton pump inhibitor for more persistent acid reflux and transfers minimally into breast milk.

Oral rehydration solutions like Pedialyte are safe for mom during breastfeeding and help prevent dehydration from diarrhea or vomiting. Staying hydrated supports milk production.

OTC eye drops — both artificial tears and decongestant eye drops — are safe during breastfeeding. Ophthalmic absorption is minimal and systemic levels are negligible.

Doxylamine (Unisom) is a sedating antihistamine used as an OTC sleep aid. It can cause infant drowsiness through breast milk and may reduce milk supply. Use sparingly if at all.

Avoid Pepto-Bismol while breastfeeding. It contains bismuth subsalicylate — a salicylate compound similar to aspirin — which can pass into breast milk and has been linked to Reye's syndrome risk.

Phenylephrine is a decongestant with a better breastfeeding safety profile than pseudoephedrine, though it's also less effective. Use with caution and prefer saline spray when possible.

Avoid pseudoephedrine (Sudafed) while breastfeeding. Even a single dose has been shown to reduce milk supply by about 25%, and it passes into breast milk.

Saline nasal spray is completely safe and is the first-line recommendation for nasal congestion during breastfeeding. It's just saltwater — no active drug ingredients.

Gas-X (simethicone) is completely safe during breastfeeding. It works only in the GI tract and is not absorbed into the bloodstream or breast milk at all.

Colace (docusate sodium) is safe during breastfeeding. Stool softeners are often needed postpartum and have minimal systemic absorption.

Throat lozenges are safe during breastfeeding. Most contain local anesthetics (benzocaine) or menthol in amounts too small to affect breast milk.

Topical pain relief products (Biofreeze, lidocaine spray, Salonpas patches) are safe during breastfeeding. Topical application results in minimal systemic absorption.

Tums is safe during breastfeeding and actually doubles as a calcium supplement. It's a great first-line option for heartburn and acid reflux.

The B6/doxylamine combination (used for pregnancy nausea) is sometimes continued postpartum. The B6 component is safe; the doxylamine component can sedate your baby. Discuss with your provider.

Witch hazel is safe during breastfeeding and is a postpartum staple for perineal care and hemorrhoid relief. Topical use has negligible systemic absorption.

Zinc lozenges are safe during breastfeeding and may shorten cold duration. They work locally in the throat and contribute only small systemic zinc amounts.
