Prednisone / Prednisolone
Prednisone and prednisolone are compatible with breastfeeding. At typical doses (up to 40mg/day), the amount reaching breast milk is small — less than 10% of the infant's own cortisol production. For short courses (like a 5-day steroid burst for asthma or allergies), breastfeeding can continue normally. For high doses over 40mg/day, consider waiting 4 hours after dosing to nurse.
Prednisone is converted to prednisolone (its active form) in the body. At maternal doses up to 20mg/day, infant exposure through breast milk is negligible. At higher doses (40–80mg/day), some experts suggest waiting 3–4 hours after dosing to minimize peak levels in milk, though even at these doses, the exposure remains well within safe limits. Long-term high-dose use should involve monitoring the infant for signs of adrenal suppression, but short steroid courses are very safe.
