Low-FODMAP Diet

The low-FODMAP diet is a therapeutic elimination diet for IBS and digestive conditions. When medically necessary, it can be followed while breastfeeding — but it should be done under the guidance of a registered dietitian, as it is complex and eliminates many nutritious foods.

The low-FODMAP diet restricts specific fermentable carbohydrates found in many fruits, vegetables, dairy, and grains. It is meant to be temporary (2–8 weeks of elimination followed by systematic reintroduction). While there is no direct evidence of harm to nursing infants from a mother following low-FODMAP, the restrictive nature of the diet and the complexity of meeting nutritional needs while eliminating so many food categories makes dietitian guidance essential. If your IBS can be managed with other strategies, discuss with your provider whether the full protocol is necessary during breastfeeding.
