Colonoscopy
A colonoscopy is safe while breastfeeding. The sedation drugs used (typically propofol or midazolam with fentanyl) clear your system quickly, and you can usually resume breastfeeding as soon as you're alert enough to hold your baby safely. The bowel prep is also compatible with breastfeeding — just stay very well hydrated.
Propofol, the most common sedation agent for colonoscopies, has an extremely short half-life and negligible transfer to breast milk. Midazolam and fentanyl, if used, also transfer minimally. Most anesthesiologists and gastroenterologists agree that breastfeeding can resume as soon as the mother is awake and alert. The bowel prep (polyethylene glycol solutions like MiraLAX or GoLYTELY) is not absorbed systemically and does not affect breast milk. Ensure adequate hydration during the prep to maintain milk supply. Pump and store milk before the procedure if you want a backup supply.
