Cocaine

Cocaine is strictly contraindicated while breastfeeding. It rapidly transfers into breast milk and has caused documented serious adverse effects in breastfed infants, including seizures, irritability, tremors, and vomiting. Do not breastfeed for at least 24 hours after use.

Cocaine and its active metabolites concentrate in breast milk. Case reports have documented infant cocaine toxicity through breastfeeding, including seizures, tremors, diarrhea, vomiting, and apnea. The AAP classifies cocaine as a drug of abuse that is contraindicated during breastfeeding. If exposure occurs, breastfeeding should be withheld for at least 24 hours โ€” though the safest course is complete avoidance. Seek help through SAMHSA's helpline (1-800-662-4357) if you need support.

Always confirm with your doctor or lactation consultant before making dietary changes while breastfeeding.
